Ihre Aufgaben
- Beratung von Pflegebedürftigen und Angehörigen zu Leistungen, Entlastungsangeboten und Versorgungsformen
- Unterstützung bei der Antragstellung und Vermittlung an passende Anlaufstellen
- Aufbau individueller Versorgungspläne
- Netzwerkpflege mit Akteuren des Gesundheits- und Sozialwesens
- Organisation von Informationsveranstaltungen, Pflegesprechstunden oder Themenwochen
Ihr Profil
- Berufliche Qualifikation im Pflege- oder Sozialbereich
- Bereitschaft zur Einarbeitung in Sozialrecht (SGB XI, SGB V)
- Empathie, Kommunikationsfreude und Organisationstalent
- Den Wunsch, Verantwortung zu übernehmen und Prozesse mitzugestalten
- Sicherer Umgang mit digitalen Medien und MS Office
